Pet Name, a 000 year old neutered male gold Pet Type, will be enjoying a holiday at Samford Pet Resort (SPR). He will be collected by office on Date IN and returned by office on Date OUT. We expect Pet Name to stay in UNIT # Order #0000 details the service and charges he will receive during his stay.

CONDITIONS OF BOARDING

General conditions

1. Owner Name guarantees that Pet Name has up to date vaccinations covering his entire planned stay at SPR.
2. There is no refund or credit for early holiday returns. All board rates are charged on a calendar day basis and the full period booked will be charged.
3. Normal and group rates are based on shared accomodation.
4. All fees are subject to change without notice.
5. Owner Name agrees to pay all costs and charges not later than the day of admittance of the pet to SPR and any additional charges incurred by Pet Name must be paid when Pet Name leaves SPR.
6. Pet Name must be dropped off and picked up within the advertised operating hours of SPR or its appointed agents. Any out of hours actions are at the discretion of SPR and may be charged for.
7. Owner Name agrees to pay all costs and charges for special services requested and all veterinarian costs for Pet Name during the time the said pet is in the care of SPR.
8. Under no circumstances will SPR, or any of its employees or agents, be held responsible for any items lost, destroyed or left behind on these premises such as toys, bedding, collars, etc.
9. Under no circumstances will SPR, or any of its employees or agents be responsible for Paralysis Tick, Canine Influenza, Cat Flu, injury, sickness, death, loss or damage of any kind whatsoever that may occur to any animal staying at SPR.
10. If all charges are not paid, SPR has the right to offer Pet Name for sale. If such sale does not secure a price adequate to cover the cost of the debt Owner Name is liable to SPR for the difference. SPR may take this course of action if not advised of a pick up date within two weeks of the advised dates.
11. By signing these conditions and leaving Pet Name with SPR Owner Name certifies to the accuracy of all information given about Pet Name. Owner Name acknowledges that the signing of this contract shall create a lien over Pet Name in favour of SPR. This lien will exist for so long as moneys are owing pursuant to this contract.
12. Owner Name agrees to give permission to SPR to allow personal information regarding Owner Name and Pet Name to be given to or received from a duly licensed veterinarian.
13. Owner Name agrees to be solely responsible for the behaviour of Pet Name while he is in the care of SPR.
14. Owner Name acknowledges that Pet Name has been treated with flea and tick prevention (e.g. frontline or advantix) prior to entry.

1. The resort manager has the right to refuse admission of any animal.
2. The owner acknowledges that if the said pet is found to be dirty, has flees or requiring grooming on arrival the resort will make all reasonable effort to contact owner, if no contact is made the resort will commence to rectify the issue and charges may apply.
3. Client acknowledges that 24hr notice is to be given if picking up pet early to ensure that departure bath and departure procedures can be completed .Resort takes no responsibility should the bath or departure procedures not be completed due to lack of notice.

Luxury accommodation conditions

1. Suites with courtyard access are closed off each evening and Pet Name will not have access to the courtyard until the following morning.
2. Pet Name must be house trained to toilet outside or use a litter tray.
3. In the event that Pet Name damages furniture, bedding or any part of the suite Owner Name may be charged for any damage incurred. If this occurs, Pet Name may be moved to the standard accommodation and the price will revert back to the standard kennel pricing.
4. SPR reserves the right to kennel Pet Name in the standard accommodation if it becomes necessary for any reason and the price will revert back to the standard kennel pricing.
5. There is a maximum of 2 family pets in any particular suite at any one time (or three very small family pets at SPR's discretion).
6. All pets are allocated to a room according to their size. SPR may move pets at its discretion.

Does Pet Name have any of the following? If the answer is yes, please describe the condition and any necessary medication.

If we find it necessary for Pet Name to have veterinary attention, every effort will be made to contact and advise you accordingly. If we are unable to contact you we will seek veterinary attention as we see fit. In any event, Owner Name will be liable for all expenses incurred.

Please note aged and intensive care pets are at a higher risk of losing weight, being off their food for a short time, experiencing anxiety, have a decline in health especially if known or unknown health issues become more pronounced or reoccur due to stress of being boarded and away from their owners.

SPR endeavour to offer the very best of care when we accept your pet for boarding. Your assistance in supplying this information and in informing us of any pre-existing conditions will help us to ensure that we provide the best possible care for Pet Name

Should you pet need vet treatment while staying at SPR

Should your pet need vet treatment during your pets stay at our resort we will.

• Try to contact you on the contact numbers that you have left( it is your responsibility to leave us contacts numbers)
• Should we be unable to contact you we will make the decision in the best interest for the pet acting with in the terms of the boarding contract.
• The pet will be taken to the vet of SPR choice unless told and noted by the client on the bottom of the boarding contract to take to a specific vet.
• All vets bills will be required to be paid by the pet’s owner as per the contract of boarding.
• Should your pet be taken to SPR’s choice of vet a fee will be charged for transport and time spent at vet. (Est. from - $40.00 transport, $25.00 labour, $15.00 van sterilisation. After hr or public holidays will incur a sure charge)
• Should you request to have your pet taken to a vet of your choice for treatment Samford Pet resort will charge a cost for transport starting at (cost depends on vet location and time spent at vet) (Est. from - $40.00 transport, $25.00 labour, $15.00 van sterilisation. • It is at the discretion for SPR to have the vet come to the resort to treat the pet and vet travel is to be paid by Their owner as per boarding contract.
